Home Care Packages

Everyone’s care needs are different. Together with My Aged Care and the Aged Care Assessment Team (ACAT) we’ll help tailor a Home Care Package to support your needs. We coordinate and manage your services for you on your behalf. There are four levels of Home Care Packages depending on the amount of support required.

Level 1: Basic care package
Level 2: Low level care package
Level 3: Intermediate level care package
Level 4: High level care package

Funding and Assessment for Home Care Packages is managed through the Australian Government – My Aged Care. To get approved for home care support services, you need to register for an assessment online or you can call My Aged Care on 1800 200 422.

After you have been assessed as being eligible you can select the provider of your choice and enter into a Home Care Agreement with them. Those with special care needs i.e. Younger people with a disability, dementia or those not met through other funding may also be able to access a Home Care Package. Personal Care

We provide a variety of personal care support services to help keep you safe, healthy and independent. We assist with all aspects of personal care; with complete respect for your privacy and dignity. This includes;

  • Showering
  • Bathing
  • Oral hygiene
  • Getting dressed
  • Going to the toilet
  • Assistance with eating
  • Medication reminders
  • Help with moving around the home – including the use of hoists and lifts.

Whether you’d like special support occasionally, once or twice a week, or you’d like us to visit every day, it’s completely up to you

Community Participation

Taking part in your community is an important part of living a healthy life. While an active social life can help prevent loneliness. We support you in all aspects of community participation. Support services can include visits to your home or companionship to an activity of your choice. Help to access community-based support groups and social activities or help with setting up phone and internet communication services so can chat with family and friends. We can provide support for just a few hours, or on a regular basis. The choice is yours!

Domestic Assistance

Getting help around the home means you can stay independent in your own home for longer. We provide a range of domestic services to keep you safe at home by helping you maintain a clean and liveable space. This could include anything from laundry and folding clothes to watering your plants and basic house maintenance. We also help ensure you continue to eat well through a healthy, balanced diet by assisting you with writing your shopping list, grocery shopping and meal preparation. When you require a service, we can’t do we will gladly refer you to someone who can help.

Respite Care

Respite care supports you and your carer by giving you both a break.  It can help give your carer the opportunity to go to the shops, exercise, run errands or even go on a holiday. Whilst giving you a break from your usual arrangement and allowing you to socialise with someone new. When your permanent carer is away, our trained professional caregivers come to your home and provide a variety of services for you or your loved one.  This includes;

  • Excising and going for walks
  • Simple companionship
  • Bathing
  • Dressing
  • Feeding and meal preparation
  • Medication reminders
  • Getting in and out of bed

Respite care can be a few hours, days or even weeks. It can be planned in advance; for instance, if your carer plans a trip or has errands to run. We are also available in emergencies.

Travel &Transport

Being able to interact with your community means that you have the transport services to get you to and from your appointments and around your community. All of Heavenly Care’s caregivers have a valid Australian drivers’ licence and can help transport you to and from things such as:

  • Medical and other health related appointments
  • Community support group or social activities
  • Shopping or other personal related appointments
  • Visiting family or having a coffee with a friend;
  • Attending functions or social activities, in and around your community

Whatever your circumstance our experienced team can offer a range of transportation and mobility options, to help you get to and from your destination as comfortably and easily as possible.

Dementia Support

A person living with dementia can feel vulnerable and confused throughout every part of their life. The effects of dementia don’t just affect the person living with the condition, but the entire family. Our experienced and informed caregivers can provide professional care for someone suffering from dementia in their own home. For someone living with dementia being able to receive care in a familiar environment provides security and peace of mind. Our knowledgeable caregivers can also gain an understanding of individual’s daily routine and create a schedule to help reduce the symptoms of dementia. Our caregivers are also a great source of advice, reassurance and support, both to you and your family members.

We can provide a wide range of services depending on your needs from assistance with meal preparation and planning, to support with tasks around the home, personal care, overnight stays and referrals to external sources of support where needed. Palliative Care Support

Even though you know the end of life is approaching for you or a loved one, you might not feel prepared and situations can easily become stressful. Our incredible palliative care team can support you in managing day to day tasks. This includes errands, preparing meals, personal care, managing medications, monitoring of your condition and well-being, and liaising with your doctor, specialists and other health professionals.

We can also provide emotional support by spending time with you or your loved one. Through reading a book together, watching TV, talking about what’s happening or listening to you share special memories or experiences. We are here to help you or your loved one stay as comfortable as possible during this final stage of life.

Post Hospital Support

For any in-home support you may need after you’ve been ill or in hospital. We work with you and your hospital to develop a plan that ensures you receive appropriate care after you are discharged. This can include anything from specialist clinical care by a registered nurse, or assistance with administering medication or treating wounds, to personal care, shopping and meal preparation. In-home Nursing

Having the freedom to live at home and receive in-home nursing is something that our caregivers understand. Our registered nurses can attend to your health issues in the comfort of your home. We assist you with staying on top of medical issues that require a professional’s attention, without you necessarily having to leave your home. This includes;

  • Wound care
  • Dressing changes
  • Insulin injections
  • Administering medication
  • Catheter care
  • Monitoring clinical signs
  • Providing specialist assistance
  • Colostomy care
  • Skilled hospice support
